Who We Are

Plan of Care (POC) is a Supported Living Services (SLS) agency whose commitment is to serve adult individuals who have intellectual and developmental disabilities (I&DD). We strongly advocate for our clients’ rights to choose how they want to live – and with whom. We make our clients know and feel that their safe choices are valued and do matter. We work to promote a culture of inclusivity for our clients’ integration within their respective communities.
